Which regime is better — Old or New?

Generally the New regime wins for incomes up to ~₹15 lakh or if you don't have heavy deductions. The Old regime wins if you can claim ₹3-4 lakh+ of deductions (HRA + 80C + home-loan interest).

Yes. If income slightly exceeds ₹12 lakh, your tax cannot exceed the amount above ₹12 lakh. So at ₹12.1 lakh income, tax is capped at ₹10,000.

What is the cess rate?

4% Health & Education Cess applies on (tax + surcharge) in both regimes.
